Vaultus is a documentation-safety tool — a quiet second set of eyes on your clinical notes. It is not a medical device, makes no clinical decisions, and never changes your notes. You keep full clinical judgment. We do not keep your patient notes. By continuing, you agree to the terms below.
Vaultus is a documentation-safety observation network — a deterministic, rules-based service that reviews clinical documentation against pre-defined safety rules and surfaces observations to you, the clinician. It is reference-only.
Vaultus is not a medical device and is not intended to diagnose, treat, cure, or prevent any disease. It supports your review of your own documentation and provides information you are able to independently review and evaluate. It does not direct clinical action, does not provide dosing or frequency directions, and is designed to operate within the FDA's Clinical Decision Support safe harbor.
Vaultus never writes into, edits, autofills, or alters your clinical note. All clinical content remains authored solely by you. Observations are reference-only.
You are solely responsible for all clinical decisions and for the content of your documentation. Vaultus does not replace professional judgment, and the absence of an observation is not an assurance that documentation is complete or correct.
The safety logic is deterministic and rules-based. Vaultus does not use machine learning or generative AI to produce safety observations.
By accepting these Terms you represent and confirm that: (a) you are a licensed healthcare provider in good standing; (b) the NPI verified during sign-up is yours; (c) you will only submit clinical documentation you are authorized to submit; (d) you are authorized to enter into the Business Associate Agreement on your own behalf, or on behalf of the practice you identified, as the covered entity; and (e) you are using Vaultus in a professional capacity, not as a patient.
Vaultus does not retain raw note text or patient identifiers (see the Privacy Policy). Where you process real patient information through Vaultus, the Business Associate Agreement governs and controls over these Terms with respect to PHI.
You will: use Vaultus only for documentation you are authorized to handle; not attempt to access or extract data of other providers; not use the service to make automated treatment decisions; and comply with applicable law and your institution's policies.
Contributing de-identified safety-signal data to improve documentation safety and research is optional and governed by a separate Data Participation choice. You may use Vaultus fully without participating.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE." TO THE MAXIMUM EXTENT PERMITTED BY LAW, VAULTUS DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. VAULTUS DOES NOT WARRANT THAT THE SERVICE WILL DETECT ALL DOCUMENTATION ISSUES.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, VAULTUS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES, OR FOR ANY CLINICAL OUTCOME, ARISING FROM USE OF THE SERVICE. VAULTUS'S TOTAL LIABILITY WILL NOT EXCEED THE TOTAL FEES YOU PAID TO VAULTUS IN THE 12 MONTHS PRECEDING THE CLAIM. This cap does not apply to Vaultus's obligations of confidentiality or under the Business Associate Agreement.
Either party may terminate per the agreed terms. The Business Associate Agreement's return/destruction provisions govern PHI on termination.
These Terms are governed by the laws of the State of California, without regard to its conflict-of-laws rules.
We may update these Terms. Material changes will be versioned and dated, and where acceptance is affected you will be asked to re-accept the new version before processing PHI.
